Improving Hypertension Management with Technology & Patient Engagement at Lake Superior Community Health Center

Lake Superior Community Health Center implemented a self-monitoring program for patients with uncontrolled hypertension aimed at improving health outcomes. The Changing Hearts team, which consisted of the Population Health Specialist, Registered Nurses, Medical Assistants, and Providers, identified necessary technology, created clinical protocols and workflows, and selected a target patient population for enrollment in their remote patient monitoring program. The initiative was associated with a 25.77% increase in the number of patients with controlled hypertension, attributed to self-management and improved communication with healthcare providers through real-time data sharing.

Located at 930 Dr. Martin Luther King Dr., inside Julian Thomas Elementary School (Door #3), the new site is classified as a Federally Qualified Health Center (FQHC), providing accessible, affordable, and comprehensive care for everyone, offering a sliding fee discount program based on income and family size. The clinic will bring much-needed access to medical and behavioral health care providers in a neighborhood that lacks sufficient health care services compared to others.

“This project represents a major milestone in expanding healthcare access in Racine,” said Dr. Mary Ouimet, CEO of Pillar Health. “We’ve heard directly from the community that they want accessible, trusted, and affordable care closer to home. That’s exactly what we’re bringing.”

U.S. Senator Tammy Baldwin visited the Lake Superior Community Health Center in Superior Thursday. As part of her “Hands Off Medicaid Tour,” she spoke with local Wisconsinites to hear their stories and perspectives on the advancing plans to make cuts to Medicaid.

People who are directly impacted by Medicaid or those who are involved in programs with Medicaid all took turns sharing their stories and thoughts: Parents of adult children with disabilities, seniors who are taking care of their parents as they struggle with dementia and Alzheimer’s, individuals who rely on Medicaid for their healthcare and their ability to stay healthy.
