Improving Hypertension Management with Technology & Patient Engagement at Lake Superior Community Health Center

Lake Superior Community Health Center implemented a self-monitoring program for patients with uncontrolled hypertension aimed at improving health outcomes. The Changing Hearts team, which consisted of the Population Health Specialist, Registered Nurses, Medical Assistants, and Providers, identified necessary technology, created clinical protocols and workflows, and selected a target patient population for enrollment in their remote patient monitoring program. The initiative was associated with a 25.77% increase in the number of patients with controlled hypertension, attributed to self-management and improved communication with healthcare providers through real-time data sharing.
