Health Center Membership

Annual Dues: Based on Annual Operating Budget

Health Center membership is open to any Wisconsin Federally Qualified Health Center as defined by Section 330 of the Public Health Service Act. Executive directors/chief executive officers of organizational members have one vote at the Wisconsin Primary Health Care Association Annual Meeting and serve as ex officio members of the Board.

Employees of organizational members enjoy full Association benefits and are eligible to participate on Association committees. Health Center staff may be nominated to serve as an at-large member of the Association’s Board of Directors and may be nominated to one of the six at-large seats on the Association’s Board of Directors.

Affiliate Membership

Annual Dues: $400

Affiliate Membership is open to any Tribal Health Clinic, Federally Qualified Health Center Look-Alike, or other entity that supports the mission and goals of the Wisconsin Primary Health Care Association but doesn’t meet the requirements for organizational membership.

Affiliate members receive reduced rates for Association conferences, training and educational offerings and subscriptions to the Association Update newsletter. Affiliate members may be nominated to serve as an at-large member of the Association’s Board of Directors.

Affiliate Members receive three complimentary individual memberships designated for organizational representatives including employees, board members, or consumers of the Affiliated Organization.
