Welcome to the Wisconsin Primary Health Care Association’s Web site, a resource for everyone who supports, promotes, uses and works in Community Health Centers. Here, you can learn about health care policies that support everyone’s access to a medical home, explore quality improvement strategies, share tactics to provide high quality and culturally competent care, engage in activities to knock down the barriers some Wisconsinites face in the health care system, talk with experts, even find a Community Health Center doctor or dentist to care for you.


 


 

WPHCA is recruiting for the 2016-17 Wisconsin HealthCorps program year. AmeriCorps members are placed with host sites across Wisconsin, including 8 community health centers, 5 local health departments, and 4 health focused non-profits. Members support patients and communities by coordinating care and resources, providing education on healthy living, and supporting community-wide health initiatives. Alumni of our AmeriCorps program are often hired on by their sites, or go on to attend graduate school or medical school. Available position descriptions can be found here

 

 

2016 Performance Excellence Award

To demonstrate WPHCA’s commitment to supporting Community Health Center (CHC) team-based performance improvement activities, WPHCA is soliciting nominations for a Performance Excellence Award, which will be conferred annually at WPHCA’s Fall Learning Session. The Performance Excellence Award will recognize and celebrate CHC performance improvement teams and the efforts those teams have made over the past year to improve the health of patients, improve patient experience with their care and/or improve the cost effectiveness of care. Nominees will be able to describe the role that team members had in achieving outcomes through planned change processes. Additionally, WPHCA recognizes that Health Center engagement and participation in WPHCA Peer Learning Networks, training, data submission, and technical assistance activities are critical to the sustainability and viability of WPHCA and our CHCs. The selected team will have demonstrated collegiality in sharing their experiences with colleagues and peers through the state and/or nation.
