Recognition and Achievements

Access Community Health Centers, Inc. are active in the National Health Disparities Collaboratives and also has been an outstanding participant in the Diabetes Quality Improvement Project for the state of Wisconsin in efforts to provide the highest quality of care to their diabetic patients and those at risk of getting diabetes.

History

For more than 24 years, Access Commununity Health Centers (ACHC) has been providing primary medical and dental care to thousands of individuals and families who have significant financial, cultural, transportation, language and other barriers to accessing care. Access Community Health Centers help people access this care by providing a sliding discount program, interpretation, case management services and assistance with transportation. In 2005, Access Community Health Centers opened their William T. Evjue Clinic on East Washington Avenue, Madison. This facility, and their South Clinic on Park Street, has given Access Community Health Centers the clinical space to more htan double the number of people they serve.
