Hotel Information: We have a block of rooms at the Quality Inn, 30600 US-2, Ashland, 715-682-5235. Wednesday night rate is $267.19, Thursday night rate is $151.99. All rooms are reserved under Lindsey Hess. When you call to make your reservation ask for a room under Lindsey Hess and change it to your name (call Lindsey Hess if you have any questions.) Please note that since the hotel has breakfast, we will not be serving breakfast at this meeting.

 

Goal of Operations Network:

The goal of this peer learning network is to provide a venue in which operations leaders at community health centers in Wisconsin are able to collaborate, learn from each other and share best practices. The network will strengthen relationships and networks as well as leverage the collective knowledge to spread strategic ideas and enhance skills.

 

Meeting Objectives:

  • Leverage the collective knowledge of the group to accelerate Health Center work related to improving the quality of Health Center operations.
  • Strengthen relationships/networks among Health Center staff.
  • Gain strategic ideas and tactical skills to use on the job.

9:00 – 10:30     A deep dive into integration and teams with NorthLakes             

Integration is a hot topic in the CHC world. In July, WI CHC leaders came together to talk about the current state of integration and identify next steps to move their CHCs further down the integration path. This session provides an opportunity get further into the details of how to operationalize those next steps. The NorthLakes team will share their approach to whole practice integration and we will have the opportunity to hear from providers and care team members about their experience and perspective on the successes, challenges and opportunities around their integration journey. Engage with the NorthLakes team to learn and understand how their experience can translate to your own!

10:45 – 11:30    Show and Tell: Team-Based Care

Whether implementing PCMH or wrestling with integration, team-based care is quickly becoming the expected approach to care. Bring your favorite tool, tip or trick for supporting teams in your health center as we take this time to share best practices. Think about a report, meeting strategy, communication tip, process etc. that you’d like to share with your colleagues.

NorthLakes recently opened a new site in Ashland, which collocates PT/OT, Behavioral Health and Medical. Come take a look at what they have done!                

 

1:30 – 2:00       Panel Management

When facing questions about physician productivity or capacity, a first step to establishing the current condition of your practice is to look at supply and demand. Kay will share some tools used for empanelment of medical providers and lead a discussion on how CHCs are looking at or assessing supply and demand.
