The Association offers three levels of membership.
Organizational Membership
Annual Dues: Based on Annual Operating Budget
Organizational membership is open to any Wisconsin Federally Qualified Health Center as defined by Section 330 of the Public Health Service Act. Executive directors/chief executive officers of organizational members have one vote at the Wisconsin Primary Health Care Association Annual Meeting.
Employees of organizational members enjoy full Association benefits and are eligible to participate on Association committees and may be nominated to the clinical provider seat on the Association’s Board of Directors.
Affiliate Membership
Annual Dues: $400
Affiliate Membership is open to any Indian Tribal Health Clinic, Federally Qualified Health Center Look-Alike, or other entity that supports the mission and goals of the Wisconsin Primary Health Care Association but doesn’t meet the requirements for organizational membership.
Affiliate members receive reduced rates for Association conferences, training and educational offerings and subscriptions to the Association Update newsletter and other notices. Affiliate members may be elected to serve a two-year term as one of four at-large representatives on the Wisconsin Primary Health Care Association’s Board of Directors.
Affiliate Organizational Members receive three complimentary individual memberships designated for organizational representatives including employees, board members, or consumers of the Affiliated Organization.
Individual Membership
Annual Dues: $75
Individual membership is open to anyone who supports the mission and goals of the Wisconsin Primary Health Care Association. Individual membership is restricted to individuals who are not employees of an organization eligible for an organizational or affiliate membership. Individual members receive reduced rates for Association conferences, training and educational offerings as well as a subscription to the monthly Association Update and other notices. Individual members may be elected to serve a two-year term as one of four at-large representatives for the Wisconsin Primary Health Care Association’s Board of Directors
